BY THIS DEED which is intended to be registered at the Administrator and Registrar-General’s Office, Roxy Building, Walpole Street, Freetown in the Western Area of the Republic of Sierra Leone, I the Undersigned ALICE MAMIE BAH of No.7 Bright Lane, Cole Farm, Murray Town –
Freetown in the Western Area of the Republic of Sierra Leone aforesaid, now or lately called ALICE MAMIE BAH, a Citizen in the Western Area of the Republic of Sierra Leone, DO HEREBY:-
- For and on behalf of myself and my relatives wholly renounce relinquish and abandon the use of my former name of ALICE MAMIE BAH and in place thereof DO ASSUME from the day of MAY, 2023 the name of ALICE MAMIE KALLON so that I and my relatives may hereafter call and refer to me not by my former name of ALICE MAMIE BAH but by my assumed name of ALICE MAMIE KALLON.
- For the purpose of evidencing such determination I DO DECLARE that I shall at all times hereafter in all Records Deeds and Writings and in all Proceedings Dealings and Transactions private as well as public and upon all occasions whatsoever use and sign the name of ALICE MAMIE KALLON as my name in place of and in substitution for my former name of ALICE MAMIE BAH.
- EXPRESSLY Authorize and request all persons at all times hereafter to designate and address me by such assumed name accordingly.
